New Member Application

The Ohio Developmental Disabilities Council (DD Council) welcomes applications for new membership. Recruitment of membership is always open so feel free to apply at any time. 


How do I apply to become a member?

You can apply using one of the following methods: (1) Request an application packet from DD Council; (2) Download, print and mail the application packet from DD Council’s website; or (3) Complete the application on-line (both DD Council's and the Governor's applications must be completed).

Method 1: Request an application packet from DD Council by mailing, calling or emailing our office.

Method 2: Download, print and mail the application packet

Download and print the both applications listed below. Send by mail to Ohio DD Council, 899 East Broad Street, Suite 203, Columbus, Ohio 43205. Here are the links to the applications: 

Ohio DD Council's Application - Print, complete and mail. 

Governor’s Office Application - This form can be completed online or you can print it an complete it by hand. If you complete it online, do NOT submit this form using the "Submit Form" button on the form. Instead, please print and mail this form along with the Ohio DD Council Application. Also, this form asks you to send a resume or brief biography to the Governor's Office, but you do NOT have to do that since the Ohio DD Council Application will already have all that information.

Method 3: Complete the application packet online

Complete your application online. Once you are finished, the application will be automatically emailed to DD Council. There will be certain parts of the online application that will require you to mail or email information to us.

Why should you become a member?

Being a member of the Ohio DD Council not only helps Council to achieve its goals, but also helps Council members learn how to be stronger advocates and to stay informed about issues that affect people with developmental disabilities.
