New Member Application

The Ohio Developmental Disabilities Council welcomes applications for new membership. Recruitment is always open so feel free to apply at any time. This page provides information on who can apply to become a member, the responsibilities of our members, and instructions on different ways you can apply.

Who can apply?

Ohio DD Council membership requires that applicants must comply with categories listed in the federal Developmental Disabilities Act. The categories are as follows:

  1. Individual with a developmental disability.
  2. Parent or guardian of a child with a developmental disability.
  3. An immediate relative or guardian of an adult with a mentally impairing developmental disability who cannot advocate for himself/herself.
  4. An immediate relative or guardian or an institutionalized or previously institutionalized individual with a developmental disability or an individual with a developmental disability who resides or previously resided in an institution.
  5. A representative of a local and non-governmental agency concerned with services for individuals with developmental disabilities.
  6. A representative of a private non-profit group concerned with services for individuals with developmental disabilities.

All candidates for DD Council membership should have experience serving on committees, boards, or organizations. Such experience concerned with persons with developmental disabilities would be especially relevant. Applicants should have first-hand knowledge about developmental disabilities in Ohio.

What are the responsibilities of being a DD Council member?

Being a member of the Ohio Developmental Disabilities Council means you will agree to do the following:


  • Attend two (2) days of meetings, six (6) times per year.
  • Attend Council's conferences and events.
  • Participate in two (2) Committees of Council which meet during the regularly scheduled Council meetings.
  • Participate in new member orientation sessions.
  • Prepare for Council and Committee meetings by reading information sent to you from Council staff.
  • Advocate for Council's Mission and Philosophy.
  • Participate in the development of Council positions on matters of public policy.
  • Have the ability to communicate either by voice, interpreter, signing, or communication device.
  • Participate in at least one (1) grant review panel for the purpose of awarding DD Council funds.
  • Read and respond to action alerts.
  • Consult with Council staff as needed.


  • Serve on various Council ad hoc groups.
  • Represent Council at selected state or local events.
  • Assume role of Committee Chair, if appointed.
  • Mentor a new Council Member.
  • Provide information to state and/or federal legislators.
  • Share information between Council and other organizations, as appropriate.
  • Serve on groups external to Council.
  • Participate in visits to Council projects with Council staff.

How do I apply?

You can apply using one of the following methods:

  • Request an application packet from DD Council;
  • Download, print and mail the application packet from DD Council’s website; or
  • Complete the application on-line.

Method 1: Request an application packet from DD Council by mailing, calling or emailing our office.

Method 2: Download, print and mail the application packet

Download and print both applications listed below. Send by mail to Ohio DD Council, 1810 Sullivant Avenue, Columbus, Ohio 43222. 

  • Ohio DD Council's Application: Print, complete and mail. 
  • Governor’s Office Application: This form can be completed online or you can print it an complete it by hand. If you complete it online, do not submit this form using the "Submit Form" button on the form. Instead, please print and mail this form along with the Ohio DD Council Application. Also, this form asks you to send a resume or brief biography to the Governor's Office, but you do not have to do that since the Ohio DD Council Application will already have all that information.

Method 3: Complete the application packet online

Complete your application online. Once you are finished, the application will be automatically emailed to DD Council. There will be certain parts of the online application that will require you to mail or email information to us. 

After you submit your application with all your supporting documents, the following will happen.

  1. The Membership Committee of DD Council will review information submitted by all applicants.
  2. Those applicants who meet membership requirements and may fill Council needs in terms of geographic diversity, ethnic and cultural diversity, representation of rural and urban areas, and so forth, will be invited to attend a group interview meeting with the Membership Committee. Those applicants selected by the Membership Committee and Council will be submitted to the Governor of Ohio.
  3. Those individuals appointed by the Governor will be notified of their appointment by the Governor's office.
